We make the time to review each request, personally,
And we will contact you as soon as possible regarding the status
of your request.

At this time, although the Foundation is growing, we can only assist residents
of Wisconsin and Jacksonville, Florida.

As the Foundation continues to grow, we will be able to expand our area.

Funding Request Forms can be found below.
Please note:
The maximum amount per grant will be $800.
Application for a grant is limited to once every two(2) years.

Our mission at the Foundation is to help women who are going through breast cancer treatment to obtain the assistance they need to focus on the job of healing verses the financial impact to their families.

To help our Board of Directors understand your request for funds please complete the appropriate application and return it to the foundation office.
Please include a budget indicating proposed use of funds.

The LeRoy Butler Foundation Grant Allocations Committee and Executive Committee will make all decisions regarding the funding of proposals.
